EXCELLENT EXAMPLE
for a
DIABETIC STATUS REPORT:
This letter was written by an area physician. It is the
best example I have ever seen of how to write a concise, COMPLETE, and
acceptable summary of a non-insulin dependant diabetic's status. It is
offered for your consideration in assisting a pilot provide the necessary
information for a type II diabetic condition. Change the specifics to fit
your patient:
Dear Dr. Terrell,
Mr. Joe Pilot has been under my treatment for Diabetes Type II and is currently taking Metformin 500 mgm twice daily. Mr. Pilot has not exhibited any on-going medical complications. He has no evidence of retinopathy, diabetic-related cardiovascular disease, diabetic vascular complications or neuropathy. He has not exhibited any signs of renal problems or hypoglycemic episodes and has not been prescribed beta blockers.
Mr. Pilot has made excellent progress in his treatment of Diabetes Type II. He has shown willingness to learn about the disease and has taken appropriate action to control its effects. He has attended nine hours instructional diabetic teaching classes here at St. Elsewhere's Hospital with success. Within 60 days of diagnosis and treatment the patient went from an A1c of 13.4 to 7.0. The patient currently exercises regularly and has a BMI of 22.4.

above, there should be no problems certifying you for a special issuance, any
class. And remember, you will need this annually even if you do not need
an FAA physical exam.
