PROTOCOL or CHECKLIST FOR
CORONARY HEART DISEASE EVALUATION
FOR ANY CLASS APPLICATION
□ 1. Six months must have elapsed since the last cardiac "event".
□ 2. Obtain hospital records (contact the hospital Medical Records department, they are usually quite helpful):
a. Hospital admission summary (history and physical)
b. Coronary catheterization report (if 'cath' done). Get a copy of the imaging on a CD disk to include with the report. (Medical records may not have this, you may be directed to the "Cath Lab" where the test was done)
c. Operative report (of any stent or surgery done)
d. Discharge summary

* First and second class applicants need a repeat angiogram (heart cath) for initial return to the cockpit. This will need to be arranged with your cardiologist early so they can plan. Remember, not before six months have elapsed from the last cardiac 'event' (like stenting, bypass surgery, etc.). This is a slightly different evaluation, call the office for details. The heart cath is usually needed only once.
HINTS: NEVER send originals, keep copies of EVERYTHING. Make a sheet of small labels with your name and date of birth, stick a label to every copy of ANYTHING you send in. It's best to submit everything, including your fresh FAA exam, at one time in one mailing to the FAA. Going through your AME's office is usually easiest and best.
